What documentation is required for a referral?
To ensure timely scheduling and insurance processing, please visit our For Providers page and select the appropriate medication form. Please include supporting documentation related to the patient’s diagnosis to help ensure a smooth process for all parties.
- Oregon Specialty Order Form.
- Demographics, including copies of your insurance cards (front and back):
- Most recent History and Physical, including supporting clinical notes to including supporting documentation outlining any previously therapies attempted and/or discontinued due to inefficacy, adverse reactions, or contraindications, intolerance, outcomes, to conventional therapy and current medication list.
- Laboratory and Imaging reports.
How do I know which order form to use?

Will your team handle prior authorizations?
Yes. Our patient access team coordinates directly with payers to verify benefits and complete prior authorization requirements before scheduling a patient.
How will I be updated on my patient’s care?
Our clinical team provides updates and documentation to the referring provider throughout the course of treatment. If any concerns or changes arise, we’ll contact your office directly.
How quickly can patients be scheduled after a referral is submitted?
Scheduling timelines vary based on medication approval and insurance clearance. However, our goal is to initiate treatment as quickly as possible, within approximately two weeks once authorization is secured. The patient will be notified once a determination is made, unless otherwise requested.
What if my patient is already on therapy elsewhere?
We can transition patients from other infusion sessions with a new referral and appropriate documentation. Please contact us for support with continuity of care.
How can I add a medication to your formulary?
Please email osi.therapies@oregonsg.com to inquire about our current formularies or to request a new medication. If you are requesting a new medication, please include any available details about anticipated patient volume. New requests are reviewed weekly by our team.
Can I request a specific product, such as the originator or a specific biosimilar?
Most insurance companies have a preferred biosimilar formulary, which can change throughout the year. We may not be able to obtain a prior authorization for a non-preferred product, so if you would like the patient to receive a specific product, please include documentation of medical necessity to support the request (such as adverse reaction to other products).
